Community Health Network Foundation is a 501(c)3 tax-exempt organization, and your donation is tax deductible within the guidelines of U.S. law. Please keep your receipt as your official record. We'll email it to you upon successful completion of your donation.

Center of Hope

For victims of violence, abuse or neglect, specially trained forensic nurses offer support in our Center of Hope sites of care. Your donation can provide clothing, comfort items, hygiene supplies and more to individuals who have had to leave an unsafe situation without any belongings.

Greatest Needs

As a community-based health system, we value our ability to act nimbly to meet the needs of members of our community when and where they need it most. Your support can lend a hand to qualified primary care, MedCheck and hospital patients who face food insecurity, unreliable transportation or other unmet social determinants of health.
